First Street Family Health (FSFH) is permanently closed as our merger with

Heart of the Rockies Regional Medical Center (HRRMC) and Salida Health Clinic is complete.

All of our patient services have ended including our website, Updox patient portal, appointment and RX requests, phones and fax numbers, etc.
Our 719-539-6637 number will temporarily remain active for final billing, medical record requests, and business operations

All appointments and patient services will now be handled through
